Parents are the primary educators of their children. Our Faith Formation Program is ready to assist you. Click on the link below to download and print the Registration Form. Please return completed forms directly  to Gayle Castellani in the Faith Formation office of the Parish Center. To download the latest Faith Formation Registration form PDF click here. Yearly registration is required in order to have the most up to date contact information for your child.
